Use this tool to find a doctor, hospital, equipment/supplies, or other type of healthcare provider. You can also search based on a medical condition. This search will return a maximum of 200 providers closest in distance to your selected criteria. For broader results, create a custom directory.

Create a Custom Directory
Use this tool to create a custom directory of providers that match your criteria. A link to your directory will be emailed to you in less than 24 hours. Select the "directory library" in this tool to immediately access a directory of Mayo Health System providers in our network.

Order a Printed Directory
Twice a year in the spring and fall, we print a directory that includes every provider in our network. To order a printed directory, complete this form.

Nominate a Provider
If you can't find the provider you're looking for in our network, you can complete a provider nomination form.

Once a week, we update our website provider demographic data (i.e. names, practice locations, phone numbers, etc.) and remove providers who are no longer participants in our network. Once a month, we add new providers to our website. Due to these changes and varying benefit plans, inclusion of a provider on this website or in our printed directory does not guarantee in-network status at the time services are rendered. To confirm provider participation, we recommend calling us at 1-800-605-8259 just prior to receiving services or ask the provider office upon scheduling an appointment.
