Accurate Information at Year-End Helps Us Serve You Better


December and January traditionally are busy months for groups who are changing administrators. To help us continue to reprice claims accurately and promptly, we need correct information.

New Groups. Please send us a completed Group Enrollment Form at least 15 days before new groups’ effective date so we can prepare our systems and staff to process claims. Along with this notification, please remember to e-mail a sample ID card to Group Enrollment or fax it to (402) 390-7262 for approval.

Terminated Groups. We need a Termination Form at least 30 days before a termination date, including the name of the new payer and run-in/run-out details, to process claims accurately.
