Modifier Guidelines

For claims involving multiple procedures, Midlands Choice applies a 50 percent reduction to second and subsequent procedures during repricing, whether or not “51” appears in Box 24D on the CMS-1500 claim form.  Therefore, additional multiple procedure reductions should not be applied during claim adjudication.
