Great Western Bank

Effective February 1, 2010, approximately 400 Nebraska and Iowa employees of Great Western Bank and Affiliates began accessing the Midlands Choice network through the Sanford Health Plan.

Providers who have direct contracts with Sanford Health Plan should submit claims to P.O. Box 91110, Sioux Falls, SD, 57109-1110. Other providers should submit claims to Midlands Choice.
