Effective Jan. 15, 2010, claims that currently pend for missing or invalid NPI information, including claims submitted by payers to Midlands Choice for "rush" repricing, will be rejected. The original claim will be returned to the provider.
For each provider (referring, rendering, service facility, billing, attending, operating, etc.) named on a claim, that provider's NPI must be included. For additional details, refer to sample UB-04 and CMS-1500 claim forms.
