The deadline for converting to the 5010 version of 837 claim files is coming soon. On January 1, 2012, all 837 claim files must be transmitted using the 5010 version. On this date, Midlands Choice will no longer accept 837 claim files in the 4010 version.
Beginning January 1, all claims must be submitted with a physical address in the Billing Provider's address loop and segment. P.O. Box or Lock Box addresses can only be submitted in the Pay-to-Provider's address loop and segment. If you have not provided Midlands Choice with a physical billing address, please do so as soon as possible to avoid claim rejections and payment delays.
In addition, billing address and facility address zip codes must be submitted in a 9 digit (zip plus four) format. Nine digit zip codes can be obtained by contacting the US Postal Service or by going to the zip code lookup on the US Postal Service website.
