Group numbers are MX0000 for Methodist and JE0000 for Jennie Edmundson. To contact Coresource with benefits, eligibility or claim status questions, call (800) 990-9058. Any claims submitted on or after August 1, regardless of date of service, should be submitted to the Midlands Choice claim address, P.O. Box 5809, Troy, MI, 48007-5809, or electronically with 47080 as the routing number. Midlands Choice will forward the repriced claim to FMH CoreSource. |
