CIGNA is significantly reducing the number of code combinations requiring supporting documentation, effective February 21, 2011.
For Modifier 59, the number of code combinations requiring supporting documentation will be reduced from 79 to four. For Modifier 25, Level 4 Evaluation and Management code combinations will be removed from the list, reducing the number requiring documentation from 74 to 57.
The current list of modifier 25 and modifier 59 code combination requirements is available on CIGNA's website.
