Coverage for employees of Pella Regional Health Center will move from Principal to Auxiant effective January 1, 2012. The group number is 1683. Call (800) 475-2232 for for eligibility, benefits or claim information. For precertification, call iProcert (866) 726-6584. All chemotherapy and radiation therapy must be pre-authorized by calling Auxiant at (800) 475-2232.
Claims submitted on or after January 1, regardless of date of service should be sent to the Midlands Choice claim address, P.O. Box 5809, Troy, MI, 48007-5809, or electronically with 47080 as the routing number. Midlands Choice will forward the repriced claims to Auxiant.
