United Security Life and Health Insurance Company is changing administrators for members who access the Midlands Choice network.
For dates of service on or after January 1, 2011, submit claims to Midlands Choice using "United Security" as both the payer and group names. The group number, USL001, will not change.
For dates of service on or before December 31, 2010, continue to submit claims to CoreSource at P.O. Box 2870, Clinton, IA, 52733-2870. New ID Cards are being issued.
Claims submitted to CoreSource with dates of service after 2010 will be returned to the provider for correct resubmission.
