Winneshiek Medical Center, Decorah, Ia., will move its coverage from Principal to Midwest Group Benefits, effective July 1. The group number is 001A61. As shown on the sample ID card, call (800) 344-3766 or (563) 382-9611 for eligibility, benefits or claim status information. For precertification, call (800) 670-7718.
After July 1, regardless of service dates, submit all Winneshiek claims to the Midlands Choice claim address, P.O. Box 5809, Troy, MI, 48007-5809, or electronically with 47080 as the routing number.
