Three Midlands Choice employee groups, 1st Financial Bank USA, Great Western Bank and Affiliates, and Siouxland Mental Health Services, access the Midlands Choice network through the Sanford Health Plan.
Providers who have direct contracts with Sanford Health Plan should submit claims to P.O. Box 91110, Sioux Falls, SD, 57109-1110. Other providers should submit claims to Midlands Choice. Claims sent to Sanford in error will be returned to providers for resubmission to Midlands Choice.
