For Healthcare Providers:

Join Our Network

Healthcare providers who participate in Midlands Choice gain access to a network membership base of nearly 300,000 people covered by healthcare plans throughout Iowa, Nebraska and South Dakota.

All healthcare professionals must successfully complete our credentialing process to be accepted as Midlands Choice network providers. Credentialing, a process which ensures that providers meet a set of membership standards, can take from several weeks to a few months, depending on the complexity of the information provided. We highly recommend reading through this entire section, including the Credentialing FAQs before beginning the process and the Application Checklist before submitting your request for network participation.

Non-Discrimination

Participation in Midlands Choice shall not be denied on the basis of gender, sexual orientation, gender identity, age, race, religion, disability, ethnic origin, national origin, patient’s insurance coverage (e.g. example, Medicaid) in which the practitioner specializes, or other factors unrelated to the criteria for participation and the ability to carry out responsibilities of the Participating Provider.

Confidentiality

Midlands Choice shall comply with all state and federal laws and regulations regarding the confidentiality of provider information.  Provider information obtained from any source during the credentialing process is considered strictly confidential and is used only for the purpose of determining the provider’s eligibility to participate with Midlands Choice and to carry out the duties and obligations of Midlands Choice, except as otherwise provided by law.  This information is shared only with those persons or organizations who have authority to receive such information, or who have a need to know in order to perform credentialing related functions.
