Provider offices can use the email address database@midlandschoice.com to notify Midlands Choice of provider demographic additions, changes, and terminations. When emailing database@midlandschoice.com the effective date of the change, as well as the NPI and/or tax identification number should be included in the email.
Types of demographic updates that can be emailed to database@midlandschoice.com include:
- Updates to a provider’s name, degree, or specialty
- Adding a new practice, billing, mailing, or remit to address
- Changes to an existing practice, billing, mailing or remit to location
- Closing of an existing location
- Adding or terminating an in-network physician or health care provider who has joined or left the practice
- Changes to the group practice name or tax identification number
Providers are encouraged to review the demographic information that Midlands Choice has on file for their tax identification number by going to Roster Report by Tax ID on the MidlandsChoice.com website. Any changes or questions regarding the information included in the roster can be emailed to database@midlandschoice.com.
