Beginning July 1, 2012, Aetna will require precertification for outpatient infusion of Enzyme Replacement drugs and Immunoglobulin (IVIG) drugs. Precertification for these drug classes may be obtained by calling 1-866-503-0857. A “Precertification Request for Injectable Medication and/or Outpatient Infusion Services” form can be located on Aetna’s secure provider website, by selecting “Aetna Support Center”, then “Forms Library”. The form can be faxed to 1-888-267-3277.
Also effective July 1, Aetna will require precertification for outpatient infusion of (Hemophilia) Blood Clotting Factor drugs. Call the number on the member’s ID card for precertification of this drug class.
