As part of the CoOportunity Health liquidation process, individual and group members have received notice as to when their CoOportunity Health coverage will terminate. All group policies have terminated as of April 14, 2015. All individual policies will terminate on August 31, 2015.
Eligibility of patients presenting CoOportunity Health identification cards may be verified by contacting CoOportunity Health at 1-888-324-2064.
