It is important that the patient’s group number and the name of the payer (not Midlands Choice) be included on each claim submitted. Doing so will assure that the claim will be repriced accurately and successfully forwarded to the correct payer for processing. The group number and payer name can be located on the patient’s identification card. Claims that do not contain this information will be sent back to the provider.
