Midlands Choice has implemented the use of CAQH applications for credentialing providers. In order to ensure that Midlands Choice has the most current demographic information for claims processing and directory purposes, it is imperative that the CAQH application is updated to include practice locations and addresses that have been added or terminated. It is also important to verify that a current certificate of insurance is uploaded to the application.
