As a reminder, Midlands Choice requires a minimum of $1 million per occurrence and $3 million aggregate of professional liability insurance coverage to meet the criteria for participation. A current copy of the Professional Liability Insurance certificate showing the amounts of coverage and the expiration date, as well as proof of participation in the Nebraska Excess Liability Fund (NELF) or Kansas Healthcare Stabilization Fund, if applicable, should be attached to the CAQH application as supporting documents.
