Claims for Bright Health’s Nebraska Statewide Access plan should be submitted directly to Midlands Choice. The group number for this plan, which is displayed on the member identification card,  is BHPNEMC. The Midlands Choice logo is also displayed on the ID card, as well as the claims mailing address of PO Box 5809, Troy, MI 48007-5809, and the payer ID 47080. 

Bright Health has indicated that they are receiving claims directly from providers for the Nebraska Statewide Access plan.  Bright Health is rejecting these claims to the provider advising to submit claims to the address listed on the ID card. 

When submitting claims for members of the Bright Health Nebraska Statewide Access plan, please remember to place the group number of BHPNEMC in field 62 of the UB-04 form and field 11 on the CMS-1500 form. Doing so will expedite the repricing of the claim and assure that claim is routed correctly.
