Midlands Choice recently added a self-service reporting tool to the MidlandsChoice.com website that gives providers the ability to generate and download a listing of the demographic information that Midlands Choice has on file for all providers listed under a tax identification number. After entering a tax identification number in the Roster Report and selecting "Search", a report appears that shows the following information:
- Provider name
- NPI number
- Address effective date
- Group practice name and type 2 NPI number
- Address
- Phone and fax numbers
- Clinic email and website
- Provider's primary and additional specialties
Once the report is generated, it can be exported to Excel. The Roster Report tool can be found by going to the "For Healthcare Providers" section of MidlandsChoice.com and selecting "Credentialing" and then choosing "Roster Report By Tax ID".
The Roster Report was created so that provider offices can easily verify a provider's effective date or review the information that Midlands Choice has on file to ensure that the information is correct and current. If a discrepancy is found, the office personnel may email database@midlandschoice.com or use the Demographic Update Form, also found in the "For Healthcare Providers" section of MidlandsChoice.com to notify Midlands Choice of the change.
