Midlands Choice can no longer send claim issues to Bright Health on behalf of the provider. For questions or help resolving issues, contact the Bright Health provider services line at 866-239-7191 for Individual and Family plans. Additional information is available here

All claim reconsiderations must be submitted through the Bright Health dispute portal located here. Bright Health has communicated that they will continue to process claims and disputes reflecting state timely filing guidelines and regulatory requirements. All claims submissions will be worked to their proper completion. 

The Nebraska Department of Insurance website houses a document addressing frequently asked questions regarding Bright Health’s exit from the Health Insurance Market Place. The document includes the Nebraska Department Of Insurance’s contact information for healthcare providers with outstanding claims with Bright Health.
